For your convenience Montgomery Surgery Center is now offering online pre-registration for your surgery.  Registration is designed to be quick and easy and completing the form should take you 10-15 minutes.  Please have your insurance information handy.

If you do not want to complete the online form, you may print and complete a demographic information form and either fax to us at 301-294-7847 or bring with you on your date of surgery. 

Due to the nature of the surgery we perform, we do not honor Advance Directives.  If you would like to know more about Advance Directives, you can access information on the state website. 

Our center does have physician partners and your surgeon may be a partner at our facility. 

You may receive Welcome letter and packet in the mail.  You will find a copy of patients rights and responsibilities in that packet.
