Before My Surgery

To expedite your check-in process, we would like to receive your registration form prior to your procedure date.  You can click on the Pre-registration link on the left of this page to complete a form online.  If you received a form in the mail, you may fax your registration form to 301-294-7847, attention Pre-Registration Department.  The Pre-Registration Department can be reached at 301-424-6901.

If you are having anything more than local anesthesia with your procedure, a member of our staff will call you prior to your surgery to collect pre anesthesia information about your health to expedite your admission process.  To reach the Pre-registration department, call 301-424-6901 or fax line is 301-294-7847.  You may access the form by clicking the link above.

You may be required to have a physical before your surgery.  Please see our pre-operative guidelines for more details.  A history and physical form should be filled out by your physician and faxed to us prior to your procedure.

If you have not recieved a call by 3:00 p.m. the day before your surgery or have additional questions please call us at 301-424-6901 between 8:00 a.m. and 5:00 p.m.

It is important for you to follow these guidelines before surgery to prevent delay or cancellation.

Food and Drink

If you are arriving BEFORE noon:
Do not eat food or dairy products after midnight the night before your surgery. 

If you are arriving AFTER noon:
You may have clear, non-alcoholic liquids up to three (3) hours before your arrival time.

Alcohol – Do not drink alcohol for at least 12 hours before your arrival.

Smoking – No smoking after midnight the night before your surgery.

Makeup, Nail Polish, Jewelry (including body jewelry) - Please remove all these items before surgery.

Clothing – Wear loose, casual, comfortable clothing to fit over dressings.

Valuables – Please leave valuables at home, we will not be responsible for lost or stolen items.

Notify The Center & Your Surgeon If:

  • You have an elevated temperature, cough, cold, vomiting or diarrhea between your last visit to the surgeon and the day of your surgery.
  • You suspect you may be pregnant.
  • You are taking: diet pills, aspirin, ibuprofen, or certain herbal medicines.  You may need to stop taking these 1-2 weeks before your surgery.  You can discuss this with your surgeon. 

What You Need The Day Of The Surgery:

  • Information To Bring – Insurance cards, photo ID, Medicare or Medical Assistance information, numbers and addresses.
  • Driver – If you are having anesthesia with your procedure, you must plan to have someone drive you home.  You cannot go home alone via taxi or other public transportation.
  • Escort – If you are having anesthesia, a friend, relative or dedicated caregiver MUST stay with you for 24 hours.
  • Legal Guardian – A legal guardian must be in the center AT ALL TIMES for children and dependent adults.
  • Care For Children – If you have young children at home for whom you normally care for, plan for someone else to care for them for at least the first 24 hours following your surgery. 
  • Interpreter – If the patient cannot read or speak English or uses sign language and interpreter is required, please let us know ahead of time.  We will make every effort to have an interpreter here for the patient.  If you need assistance arranging for an interpreter, please contact the Scheduling department 301-424-6901 as soon as possible.
